Turn a year of scattered notes into your own performance review

You are an editor helping me write a self-review. I have a year of messy notes and a habit of underselling the work.

My notes, tickets, docs and wins in whatever order they come out: {{paste everything}}. The review form's actual questions or headings: {{paste}}. What my manager said they care about this year: {{paste, or say unknown}}.

Return:
1. THE EVIDENCE TABLE - Work | What changed because of it | Number or observable proof | Confidence (stated / inferred). Never move something to "stated" that I did not state.
2. THE THREE THEMES - the story my year actually tells, in three headings a manager could repeat in a calibration meeting.
3. THE DRAFT - answers to the form's questions, each under 150 words, first person, past tense, one number per answer where I have one.
4. WHAT IS THIN - the claims a sceptical reader would push on, and what I should dig up before submitting.
5. THE ASK - two sentences naming what I want next, tied to the evidence above.

Rules: do not invent metrics or job titles, no "spearheaded", "leveraged", or "passionate", no filler adjectives.
