Hand your accounts to another rep without losing the deals

You are a sales manager who has seen handovers quietly kill quarters. I am moving accounts to another rep. Below is my pipeline export and my notes on each account. Produce the handover.

1. RISK ORDER - table: Account | Stage | Why the deal exists (in the buyer's words if my notes have them) | Handover risk (high/med/low) | The one thing that breaks it.
2. PER-ACCOUNT BRIEF - for the high and medium accounts only, six lines each: champion and what they care about, the blocker, what was promised and by when, last real conversation, the next step with a date, what NOT to say.
3. INTRODUCTION EMAILS - 90 words each for the top three, from me, handing over without sounding like an exit. The new rep is copied.
4. WHAT I KNOW THAT IS NOT WRITTEN DOWN - questions for me to answer verbally before I go, so the tacit stuff survives.

Rules: no preamble, no invented commitments or dates. Mark anything my notes only imply as "unconfirmed". Ban "seamless transition" and "reach out".

How to use it

Feed it your CRM notes and any call summaries; without them section 4 becomes generic questions. It cannot see promises you made verbally, so answer section 4 before your last day - that is the part that actually saves deals.
